Testing and treatment for a multinodular goiter in NYC is often combined with a screening for thyroid cancer. A “goiter” is an enlarged thyroid gland. As the thyroid grows in size, it may lead to the formation of thyroid nodules (known as a multinodular goiter). This thyroid condition can cause noticeable lumps around the throat and may interfere with your breathing, swallowing, or speech. Multinodular goiters can interrupt the balance of thyroid hormones and contribute to fatigue, weakness, muscle aches, and other unwanted symptoms. What Is A Multinodular Goiter?

A multinodular goiter occurs when the thyroid becomes enlarged and multiple nodules develop. These nodules are small lumps that appear on the thyroid. A goiter may continue to grow in size and, in some cases, may even begin to grow downwards into the chest. As the thyroid enlarges, it can compress local structures like the trachea (windpipe) and the esophagus (food pipe), which can cause noticeable symptoms like trouble speaking or swallowing.

Multinodular goiters can be either toxic or nontoxic. Toxic multinodular goiters produce too many thyroid hormones – a condition known as hyperthyroidism. Nontoxic goiters do not cause any hormonal imbalances.

What Causes Multinodular Goiters?

There are numerous factors that can contribute to the formation of a multinodular goiter, including:

Overactive thyroid (hyperthyroidism)
Underactive thyroid (hypothyroidism)
Iodine deficiency
Autoimmune conditions like Hashimoto’s disease or Graves’ disease
Age (older patients are at a higher risk)
Family history of goiters or multinodular goiters
Family history of thyroid nodules or other thyroid diseases

What Are The Symptoms Of A Multinodular Goiter?

Multinodular goiter symptoms vary from patient to patient. Most smaller goiters are asymptomatic. Patients with larger nodular goiters may experience difficulty speaking, swallowing, and breathing; this is because the goiter can compress the windpipe or food pipe. Noticeable lumps around the throat or a full feeling in the neck may also be present. Symptoms of nodular goiters are difficult to detect without a professional evaluation by a qualified thyroid specialist.

How Is A Multinodular Goiter Diagnosed?

Larger goiters may be detected during a physical exam by checking for lumps around the neck and throat. Ultrasound imaging can also help diagnose a nodular goiter by taking a digital image of your thyroid to see if nodules are present. If nodules are detected, a biopsy will be performed to determine if the growths are cancerous. A small sample will be removed from the nodule and sent to a pathologist for further testing.

Thyroid Medication
Medicines may be considered for smaller multinodular goiters that are not cancerous. Thyroid medication can correct hormonal imbalances of the thyroid. Some medicines are designed to supplement thyroid hormones (for underactive thyroids) and other medications can lower the amount of thyroid hormones (for overactive thyroids). This treatment may help reduce an enlarged thyroid and manage symptoms.
Thyroid Surgery
Surgical removal is an option for nodular goiters that are cancerous or have not responded to medication and radioactive iodine. Thyroid surgery involves removing all of the thyroid gland (full thyroidectomy) to resolve symptoms. Learn more about thyroid surgery and treatments by clicking here.

What Can I Expect Following Multinodular Goiter Removal?

Thyroid surgery patients require 1-2 weeks off to recover following treatment. A liquid diet may be recommended for the first week or so because it may be hard to swallow. Our thyroid surgeon will prescribe pain medication to reduce any discomfort. Patients should avoid vigorous activities and exercise until permitted by our thyroid doctor. Following your multinodular goiter removal, you may be given a calcium course to avoid low calcium levels in the blood (known as hypocalcemia).
